Hello everyone! I am curious to see how other libraries in Tennessee address their Title VI surveys/forms to their patrons when creating new library card accounts. We use the print form attached and ensure new patrons that they are not obligated to respond if they do not feel comfortable; however, we occasionally have patrons who are extremely put off by them, as they view the survey itself as othering or discriminatory. Does your library use a similar print or digital form for Title VI, and if so, how do you explain them to your patrons? Are there other ways that you have found to fulfill your Title VI obligations while still accounting for the patrons' comfort? Thank you in advance! -- *Hayley Clanton* Technology & Marketing Librarian TLA/ALA Member Shelbyville-Bedford County Public Library 220 South Jefferson Street, Shelbyville, TN 37160 (931) 684-7323
