We invite you to participate in a survey designed to explore the role of public libraries in providing access to health-related information and services, both before and after the COVID-19 pandemic. To participate, you must be 18 years of age or older and employed by a public library in Tennessee. The survey should take approximately 15 minutes to complete. Participants who complete the survey will have the option to enter their email address through a separate form to receive one of 200 $25 Amazon gift cards.

Participation is voluntary and participants will remain anonymous. No identifying information will be collected that could connect an individual with their survey responses. Raw data will only be accessible to the project investigators. The survey results will be used in an aggregate manner when disseminated in publications or presentations.
