I can't help you on the first, but can tell you what we did for the second. It's not so much what we put on the plan, but what we did with it when it was in the editing stages. After I'd written it up to the best of my ability and had my staff check it for holes, I took it to the fire chief and police chief here in town and had them look over it, make suggestions, and additions. They made *several* enlightening suggestions (I hadn't even thought to include no cell phones or radios in the case of a bomb threat, for example). After I'd run it to my board for a look over, I brought both chiefs copies of our plan to have on file. Hope that's helpful.
