Someone out there called me late last week (while I was traveling back and forth to the Nashville area) and requested a W-9 form so that her institution could pay for her conference registration.   I failed to save the name and institution.  If you are that person, could you please reply to this message so that I can email you the W-9 form?   Thank you for your understanding!
