Just a reminder that we have a couple of deadlines coming up at the end of this month.

 

Form 472, Funding Year 2017 (Funding Year 2017-2018)

                Deadline – October 28th

 

For those of you who request reimbursements, the last day to request a reimbursement for Funding Year 2017 is October 28th.  You will need a total of all eligible services per service provider and your PIN.  The form is not in the EPC Portal and is located at: https://www2.sl.universalservice.org/bear/login.aspx

 

Please keep in mind that you should not be asking for more than you spent on the service, even if it is less that what you were funded.  Also, you cannot ask for more than you were funded.

 

Form 486, Funding Year 2018 (Funding Year 2018-2019) – if received on or before July 1, 2018

                Deadline – October 29th, or 120 days after you receive your Funding Commitment Decision Letter, whichever is later

 

For those of you who have received your Funding Commitment Decision Letter, your deadline for Form 486 is as follows:

                FCDL received on or before July 1, 2018 – deadline to file Form 486 is October 29th

                FCDL received after July 1, 2018 – deadline to file Form 486 is 120 days after receiving the FCDL (calendar days, not business days)

 

The form is available in the EPC portal - https://portal.usac.org/suite/

 

Form 470, Funding Year 2019 (Funding Year 2019-2020)

                No deadline set yet

 

The window is still open to file your Form 470 for Funding Year 2019.  There has been no deadline set as of yet, as Form 471 has not been released yet.   This form is also in the portal - https://portal.usac.org/suite/
