The enrollment certification form has been revised. You no longer have to list your classes!!!

 

If you have turned in the old enrollment form for summer or fall already there is no need to re-do this form. We will continue to process the old form as normal. For future enrollments please use the new form.

 

Also, if you have the old form bookmarked on your PC please make a note of the new form and use it in the future. Hopefully this form will be easier and give you some basic information regarding your enrollment.
