Hey Sue B!
We have sign-in sheets that have the race/ethnicity and gender categories so that each attendee can identify themselves by checking the appropriate boxes.  We have language on the sheets that the information is being collected for federal/state reporting purposes and is voluntary.  Although it is voluntary, we have our staff strongly encourage participates to identify themselves, but again it is optional.

Would you verify how this info is gathered in your state?
As you gather information from clientele who are attending events (sign-in sheet or card), do you have an "optional" place for them to identify or list race and gender?
